At work we do quarterly birthdays and order a cake from Costco, ALL THE TIME. It has been a few months since I have ordered a cake myself but we had a retirement party and I used the same order form that you have a link to on this page and a colleague dropped off the form and later picked it up. However, when he went to drop the form off he was told that the form he had is an old version that they no longer use. He said the new one was very different from the old one but he didn’t bring a copy back. I have been searching for close to a hour online to find the new order form but I got nothing! Can you please let me know if there is a new one or if some Costco’s do things differently than others? The Costco I’m referring to is in Utica, MA.

The order form might also differ from location to location, so what we suggest you do is look for the contact information of the location you’re interested in using their store locations page.
